ALEX P. ACEVEDO, DDS:

Dr. Acevedo practices general dentistry in Westminster.  He graduated from University of Iowa College of Dentistry in 1982 and served his residency at the University of Iowa Hospitals and Clinic in 1983 receiving his degree in hospital dentistry.  He is a member of the American Dental Association, Colorado Dental Association, Metropolitan Denver Dental Society, International Congress of Oral Implantologists, and the American Academy of Dental Group Practices.

LISA M. AUGUSTINE, DDS:

Dr. Augustine graduated from UCLA in 1992 with a B.S. degree in Economics.  She then returned home to earn her D.D.S. degree from the University of Colorado.  After graduating in 1996, she completed a one-year A.E.G.D. residency at the University of North Carolina.  Since 1997, she has practiced general dentistry in Aurora with her partner and father, Dr. Ron Yaros, and has been a member of the ADA, CDA, MDDS, and AAID. She completed a preceptorship in dental implantology at the University of Texas, San Antonio in 1999. Additionally, Dr. Augustine was a delegate to the CDA House of Delegates in 2000 and 2002.

DOUGLAS B. BERKEY, D.M.D., M.P.H., M.S.:

Dr. Berkey is Dental Director for Total Longterm Care of Colorado which provides comprehensive dental services to over 1400 program participants in Denver.  He is an American Dental Association (ADA) Consultant to the Council on Access, Prevention, and Interprofessional Relations (CAPIR), and has previously served as an ADA delegate to the Joint Commission on Accreditation of Health Care Organizations (JCAHO).  Since 2006, Dr. Berkey has served as a member of the Governor’s Dental Advisory Committee (State of Colorado).  He is also a professor at the University of Colorado Denver School of Dental Medicine where he received the Rocky Mountain Study Club Professor of the Year Award and was Chairman of the Department of Applied Dentistry for eleven years.  Dr. Berkey serves as Editor-in-Chief of Dental Abstracts. He was a Pew National Dental Leadership Fellow, William Fulbright Scholar Finalist and recipient of the ADA Geriatric Oral Health Care Award.  Dr. Berkey has served as the Colorado delegate to the Council of Faculties at the American Dental Education Association (ADEA), International/American Association for Dental Research (IADR/AADR) group president, and Fédération Dentaire Internationale (FDI) section vice-chair.  Dr. Berkey has owned three separate dental practices and has been a private dental practitioner in Colorado, Minnesota, and Idaho.


RICHARD L. CALL, D.M.D., M.S.:

Dr. Call graduated from Washington University, School of Dental Medicine in 1975 and has been practicing General Dentistry in Thornton, Colorado for over 33 years.  Concerned with access to care issues, he completed a Masters Degree from the University of Colorado, School of Medicine Health Administration Program in 1977.  He served on the full-time faculty of the CU School of Dental Medicine for over 20 years while maintaining his practice in Thornton, developing a number of programs to improve access to care.  He continues to teach part-time and is a member of the ADA, CDA, MDDS, and is a Fellow in the International College of Dentists and the Pierre Fauchard Academy.  He is active in international humanitarian dental projects, serves as a consultant to the Colorado State Board of Dental Examiners and is a Technical Advisor to the “Cavity Free at Three” collaborative effort of six Colorado health funders.

CYNTHIA A. EVANS:

Ms. Evans is an executive professor of management at the Monfort College of Business at the University of Northern Colorado. She earned a Bachelor of Arts degree in journalism with a minor in economics from UNC and a Masters of Business Administration from the University of Denver. She was most recently Xcel Energy's state vice president for Colorado, Wyoming and Arizona and is currently on the board of the Colorado Nonprofit Association and the board of Mountain States Better Business Bureau.

DENISE K. KASSEBAUM, DDS, MS:

Dr. Kassebaum currently serves as the Dean of the University of Colorado Denver School of Dental Medicine.  She received her dental degree from the University of Missouri - Kansas City and her Masters degree in oral radiology and oral medicine from the University of Texas Health Sciences Center at San Antonio.   She is a diplomate of the American Board of Oral Medicine and a fellow of the American and International College of Dentists, and the Pierre Fauchard Academy.  Dr. Kassebaum completed a fellowship in the Executive Leadership in Academic Medicine (ELAM).  She holds leadership roles in the American Dental Education Association on the Council of Deans and Chairs the Center for Educational Policy and Research Advisory Committee.  She is a member of the American Dental Association, the Colorado Dental Association, and the Metropolitan Denver Dental Society.

 

C. DAVID KIKUMOTO:

Mr. Kikumoto is the Founder and Chief Executive Officer of Denver Management Advisors, Inc., one of the leading compensation and healthcare consulting firm in the Rocky Mountain Region.  He received his B.S. in Accounting from the University of Utah, completed the Executive Development Program at the University of Chicago, and the National Association of Corporate Directors Training in Denver, Colorado.  He has served as the President of Anthem Blue Cross and Blue Shield of Colorado, Chief Executive Officer and Vice Chairman of Rocky Mountain Healthcare and CEO of Blue Cross and Blue Shield in Colorado, Nevada, and New Mexico, and served as Executive Vice President of Regence Blue Cross and Blue Shield of Utah.  Mr. Kikumoto currently serves as chair of Delta’s Compensation Committee and serves on Delta’s Professional Services Committee.  He currently serves as Chairman of the Board of the National Volunteers of America.
 

VICTOR LAZZARO, JR.:

Mr. Lazzaro is a Managing Director with Volante Capital, LLC and TIVIS Capital, LLC, private equity firms with headquarters in Denver. With over 20 years of senior management experience in the health and managed care fields, he has held positions responsible for marketing, finance, administration and healthcare delivery.  Previously, Mr. Lazzaro was president and CEO of United Healthcare Corporation - Mountain States (UHC).  He is an active board member of a number of charity and business organizations.  Mr. Lazzaro holds a Master of Business Administration degree in finance from the University of Southern California and a Bachelor of Arts degree from Washington and Jefferson College in Pennsylvania.

B. LARAE ORULLIAN:

Ms. Orullian is Chair of the Board of Directors of Frontier Airlines and Vice Chair and Chair of the Audit Committee of Frontier Holdings, Inc.  Ms. Orullian most recently served as President of Guaranty Corporation and Vice Chair of the Board of Directors of Guaranty Bank and Trust Company.  Ms. Orullian led the formation of Women’s Bank of Denver and served as President, Chief Executive Officer, and Chair of the Board of Directors from 1977 to 1995.  She is a graduate of the ABA American Institute of Banking and the Ohio State University Graduate School of Banking, majoring in real estate finance and mortgage lending.  She was also awarded a degree of Doctor, Humane Letters, from Whittier College.  Ms. Orullian currently serves as a member of the Board of Directors and Chair of the Investment Committee for the Holladay Bank and Trust Company in Salt Lake City, Utah, as well as a member of the Board of Directors for Lange Golf Company in Palm Springs, California.  She is an active volunteer serving on the Board of Directors for Colorado Public Television - Channel 12, Past Chair and National President of Girl Scouts of the USA, First Vice Chair of the Board and Chair for the Finance Committee for World Association of Girl Guides and Girl Scouts, London, England, Colorado Supreme Court Discipline Hearing Board, member of International Women’s Forum, a board member of the Women’s Foundation of Colorado and member of Committee of 200.

KATHRYN A. PAUL:

Ms. Paul currently serves as the President and CEO of Delta Dental of Colorado.  Prior to this position Mrs. Paul served as President, Group Operations West, for Kaiser Permanente.  Mrs. Paul received her B.A. Degree in Sociology from the University of California and her Master of Public Health from the University of California, Berkeley, where she majored in Hospital Administration.  She also completed the Advanced Management Program in 1984 at Harvard University.  Mrs. Paul is the past Chair of the Board of Directors of the Federal Reserve Bank of Kansas City, Denver Branch. She currently serves as a Director of the Colorado Trust, the Denver Art Museum, and the Denver Metro Chamber of Commerce. 
 

DONALD S. SAFER, DDS, MS:

Dr. Safer practices endodontics in Denver and Greenwood Village.  He’s a clinical consultant at the University of Colorado School of Dentistry and a clinical consultant at the VA Hospital.  He graduated from the University of Michigan in 1968 with his Doctor of Dental Science degree and in 1971 with his Master of Science degree.  Dr. Safer received his Diplomat of the Board of Endodontics in 1975.  He is a member of the American Dental Association, American Association of Endodontics, Ralph Sommer Study Club, Alpha Omega, Metropolitan Denver Dental Society, Rocky Mountain Endodontic Society, and Excellence In Endodontics Study Club.  He is a member of Volunteer Dental Service – Morey Jr. High School - MDDS, Allied Housing board member, Crohn’s & Colitis Foundation Board Member, and PSC Foundation Member and professional consultant.
 

GAIL S. SCHOETTLER:

Dr. Gail Schoettler serves on the boards of AspenBioPharma, Inc., Pepper Ball Industries, Masergy Communications, Delta Dental of Colorado, The Colorado Trust and several non-profit boards.  She has been a U.S. Ambassador, Colorado’s Lt. Governor and State Treasurer, Douglas County school board president, and a successful businesswoman.  She has a BA in economics from Stanford University and MA and PhD degrees in African History from the University of California.  Among her many awards is the French Legion of Honor (France’s highest civilian award).  An enthusiastic hiker, she conquered her fear of heights and climbed all 56 of Colorado’s mountains over 14,000 feet high.

MARILYN E. TAYLOR:

Ms. Taylor retired in 2001 from her position as vice president of human resources for Excel Energy.  During the 20 years she has spent in the Denver area, she has served on the boards of the Colorado Ballet, The Denver Zoo, The Women’s Foundation of Colorado and the Adult Learning Source.  Since her retirement, she has served on the board of the Boys and Girls Clubs of Metro Denver and the board of Rocky Mountain Public Broadcasting – Channel 6.  Among the numerous awards she has received are the YWCA Woman of Achievement Award, the Frank Carr Community Service Award, and the 1997 Outstanding Business Woman of the Year.

OLIVIA M. THOMPSON:

Ms. Thompson currently serves as the Chief Financial Officer for AlloSource, a non-profit medical device company.  Prior to AlloSource, Ms Thompson served as Vice President, Corporate Excellence and Corporate Controller for Molson-Coors.  Her experience includes financial and general operating aspects of business operations with expertise in strategic planning, business process improvements and governance. She holds a Bachelors degree in Sociology, a M.S. in Public Administration, and a M.B.A. in Accounting/Finance.  She is currently Board Chairwoman elect for the Board of Trustees for 9 News Health Fair and is a member of the Executive Leadership Council.

CHERYL A. WENZINGER:
Ms. Wenzinger is a retired Deloitte & Touche audit partner.  Her auditing career was focused in Health care, insurance and real estate.  She currently serves on the board of directors and chairs the audit committees of Verenium Corporation, a publicly held bioscience company, and of Banner Health, a large health care system based in Phoenix.  She is a board member of the University of Northern Colorado Foundation and she also serves on the Deloitte & Touche retired partners board. Ms. Wenzinger was the 1999-2000 president of the 8,300-member Colorado Society of CPAs and a member of the AICPA Council from 1998-2002.
