How To Become A Broker of Record
Become a Broker of Record for an Employer Group
Include these documents with your first sold Employer Group Master Application:
• W-9 Form
• Producer Fee Agreement
• Copy of Producer License
• Copy of Agency License if a TIN is provided on the Producer Fee Agreement and W-9 form
If you have never received commission payments from DDCO you are not appointed.
Become a Broker of Record to sell Delta Dental Individual & Family Plans
You must first be appointed with DDCO in order to sell our individual & family plans. The appointment process can take up to 15 business days before receiving a broker/vendor number.
In order to receive a broker/vendor number you need to submit a copy of the following documents to brokerappointments@ddpco.com or fax to 303-741-4233.
• W-9 Form
• Producer Fee Agreement
• Copy of Producer License
• Copy of Agency License if TIN is provided on W-9 and Producer Fee Agreement
• ACH Form for direct deposit commission payments

Submit a completed Master Application for the Employer Group sold. Under the “Producer” section of the Master Application be sure to include your broker/vendor number assigned to you by DDCO.
